Project 03. The Evidence Pack

A different regional insurer shipped its claims-drafting assistant to its motor team after a two-week demo that went well: the model cited the right clause in every example the demo covered. Nobody had run it against a sample large enough to bound the error rate, priced the p90 letter instead of the average, or drawn a boundary around the tool that read customer attachments. Six weeks after launch, an adjuster's injected PDF got the assistant to draft a letter quoting the wrong exclusion; the letter reached the customer before anyone recognised the pattern, and it cost the firm three weeks of legal review and a complaint the regulator ended up seeing. The demo had looked identical to the failure.

Illustrative composite. The insurer, the injected PDF and the regulator's involvement are a composite of engagements the course draws on, not one company's record.

Harbor Mutual's steering committee meets in three weeks to decide whether the claims-correspondence pilot goes live for the motor team. The design is done: the ADR pack and the reference architecture from project 2 sit in the Dossier. What the committee does not have is evidence. The legal director wants to know how a misapplied clause will be caught before a customer sees it; the CFO wants a per-letter number she can defend and a twelve-month total; the CISO wants to know what happens when a claimant's attachment tells the assistant to do something. You will produce the three documents that answer them from the inputs below, and end with a memo that says go, conditional go or no-go, in writing, with the evidence that decides it. You hand over the evidence pack for your case: the eval plan, the cost model and the threat model, ending in a written go/no-go for the pilot.

Brief

The client. Harbor Mutual, a regional property-and-casualty insurer, about 1.2 million policies, EU and UK operations, an on-prem document archive plus a cloud tenancy in an EU region. The assistant drafts claim correspondence grounded in policy wording and proposes adjuster visits through a booking tool. Adjusters review and send every letter. Every design number in this project is an illustrative composite from the course's running case; the prices are list prices from the Anthropic pricing page, observed 2026-09-09.

Illustrative composite. The budgets and constraints below are the course's modeled values for Harbor Mutual, carried from the earlier lessons; they are not a real insurer's figures.

Constraints carried from the Dossier. From 01-nfr-budgets.md: misapplied or hallucinated clause at most 1 in 100 drafts; required disclosures present in 99 of 100; adjuster-acceptable without rewrite in 80 of 100; p95 time to first token under 1.2 s; cost per eval run under $50. From 02-constraints.md: customer data may not leave the EU tenancy; a customer's data reaching anyone but that customer is a reportable incident; adjusters will not accept an approval click on every action. From 05-agent-spec.md: a route call, a draft call, a booking loop of at most two steps on the letters that need a visit, an output guard. From 06-integration-spec.md: every tool call runs under the adjuster's identity through the gateway.

Input 1: golden-set sample. Twenty of the three hundred items as the claims quality lead delivered them: ID, stratum, claim type, the input bundle in one line, the expected output or action, and the grader the eval plan should assign. Use them to write the spec, the rubric and the slice rules; do not grade them by hand.

Steps

The go/no-go memo under Example Files is worked in full; step 3's control matrix has its first two rows worked below — fill the third yourself, then check; steps 1, 2 and the rest of step 3 stay open, built from your own golden set, forecast and tool list.

  1. Write 07-eval-plan.md from lesson 10's template. Specify the full 300-item set from the sample: strata and sizes, constructed slices with a tolerance each, item schema, owner. Write the rubric with a grader per criterion, starting from the grader column. Pre-register the judge validation threshold and show the confidence-interval arithmetic for your slice size. Write the CI gate with the booking path's pass^4 rule and the cost of one run at the linked prices, then the online sampling rate and the refresh cadence.

  2. Write 08-cost-model.md from lesson 11's worksheet. Build the task tree from the tool list and the agent spec (route, draft, booking loop, guard, judge), fill the worksheet with your token estimates, apply retries and the regional uplift, and report the mean and the p90 letter. Model twelve months under base, high and an adverse case defined as your own design with one lever broken. State the fixed lines, the TCO, the self-host break-even as the formula with badged numbers, the peak tokens per minute from the forecast, and the gateway controls.

  3. Write 09-threat-model.md from lesson 12's template. Draw the trust-boundary DFD for the tool list as a mermaid flowchart with zones and dashed crossings. Run the trifecta check and record cut or gate per leg. Fill the control matrix with at least ten rows carrying OWASP LLM, Agentic and ATLAS IDs, an owner and evidence. Write the red-team plan with a critical class and a gate. G-019 becomes both a constructed eval item and a red-team family; say so in both documents.

Two control-matrix rows, worked from the tool list above:

Tool / riskOWASP / Agentic / ATLAS IDControlOwnerEvidence
read_attachment, mediumLLM01:2025 Prompt Injection; ATLAS AML.T0051Reader emits a structured flag only, never the attachment's raw text, to any downstream call; adjuster review; online judge on the attachment stratum at 100%Platform leadG-019 red-team family, ASR reported per campaign
send_letter, highLLM06:2025 Excessive AgencyNot granted to the agent at all; only a human token can call itArchitectAgent's tool allow-list audit; absent from the list
  4. Write go-no-go.md, one page. Name the decision, quote the evidence from the three documents that decides it, give every condition an owner and a date, and state what reopens the decision. The model memo under Example Files shows the shape; your numbers must come from your own documents.

The three documents feed one decision. The picture below shows what each contributes and where a no-go comes from; look for the arrow a single critical failure follows.

diagram
drawing
Each evidence document contributes one class of criterion to the memo, and any critical-class failure routes straight to no-go regardless of the others.

A critical failure (a superseded clause cited, data leaving the claim's contact) ends the decision before cost is discussed; an open budget with an owner and a date is a conditional go; a budget the evidence says cannot be met is a no-go however good the rest looks.

Acceptance

  • 07-eval-plan.md specifies 300 items with strata matching the 55/30/15 claim-type split, at least three constructed slices with a tolerance each, and G-018 and G-019 in zero-tolerance slices.
  • Every rubric criterion names its grader and its reference material; the clause criterion is split into a code lookup (existence, version) and a judge call (applicability).
  • The judge validation pre-registers a lower-bound agreement threshold with the standard-error arithmetic shown, and forbids relabelling to fit the judge.
  • The CI gate states the lower-bound rule, slice rules, pass^4 on the booking path, the paired-regression rule, the waiver path, and the dated cost of one run.
  • 08-cost-model.md prices every call in the task tree, applies retries and the regional uplift, and shows the mean and the p90 letter with the arithmetic.
  • Twelve months are modelled under base, high and a broken-lever adverse case, fixed lines separate from model spend, with a TCO for each; the break-even is a formula with utilisation in it and every GPU number dated or badged.
  • Peak tokens per minute are computed from the forecast and compared with the provider's published limit; caps, alerts and attribution tags are listed.
  • 09-threat-model.md has a mermaid DFD with zones and dashed crossings, a trifecta table with a decision per leg, and at least ten matrix rows with OWASP LLM, Agentic and ATLAS IDs, an owner and evidence each.
  • The red-team plan names a zero-tolerance critical class, a gate, the families with counts, the runner and the campaign cadence, and routes every successful attack into the golden set; send_letter is absent from the agent's allow-list and the memo cites that.
  • go-no-go.md names one of the three decisions, quotes numbers rather than restating documents, gives every condition an owner and a date, and no number in the pack lacks a dated source or a composite badge.

Rubric

CriterionWeakStrong
The eval plan is a contractA rubric and a judge, threshold as a point estimate, no validation protocolBinary criteria with graders matched to form, a pre-registered lower-bound threshold with the arithmetic, adjudicated validation, a gate with a waiver path
The cost model is auditableOne model call priced, one scenario, GPU prices quoted as factThe full task tree, mean and p90 shown, three scenarios with a broken-lever adverse case, break-even as a formula with badged numbers, gateway controls
The threat model is structuralA list of OWASP IDs with "input validation" as the controlBoundaries drawn, the trifecta decided per leg, controls that remove capability, every row with an owner and evidence, a red-team plan with a critical class
The memo decidesRestates the documents and recommends "proceed with caution"Names go, conditional go or no-go; quotes the deciding numbers; every condition has an owner and a date; says what reopens the decision
Sourcing and honestyUnsourced numbers; a rounded pass rate; a risk without a controlEvery number linked or badged; the lower bound reported as it is; every accepted risk has a control and a signature

Key Takeaways

  • Evidence, not design, is what a committee signs; produce the three documents before the meeting, not the slide.
  • Any critical-class failure ends the decision before cost is discussed.
  • An open budget with an owner and a date is a conditional go; a budget the evidence says cannot be met is a no-go.
  • Quote the lower bound, the p90 and the critical-class result as they are; the memo's credibility is its arithmetic.
  • Every accepted risk in the memo carries a control and a name.

What You Have Now

The evidence pack for your case: the eval plan, the cost model and the threat model, ending in a written go/no-go for the pilot. It lives at dossier/07-eval-plan.md, dossier/08-cost-model.md, dossier/09-threat-model.md and dossier/go-no-go.md, on top of the ADR pack and reference architecture from project 2. It closes the Prove stage of Frame / Decide / Prove / Run. The Run stage builds on it directly: the compliance annex in lesson 13 cites the matrix rows as evidence, the runbook in lesson 14 inherits the gate thresholds and the capacity numbers, and the rollout plan in lesson 15 turns the memo's conditions into stage-gate criteria.

**Prepared by:** [architect] · **For:** steering committee, [date] · **Documents:** 07-eval-plan v1.0, 08-cost-model v1.0, 09-threat-model v1.0

## Evidence that decides it

| Question | Evidence | Source | Result |
|---|---|---|---|
| Will a misapplied clause reach a customer? | Superseded-policy slice: 0 failures of 20; clause criterion validated at lower-bound agreement 0.81, kappa 0.71 | 07 §4, §6 | Critical class clean |
| Is the assistant good enough to draft? | Pass rate 0.84 on 300 items, lower 95% bound 0.798 vs threshold 0.80 | 07 §6 | Open (condition 1) |
| Does the booking path work repeatably? | pass^4 = 0.87 on the 60-item booking slice vs 0.85 | 07 §5 | Met |
| What does a letter cost? | Mean $0.035, p90 $0.076; regional endpoint uplift included | 08 §1 | Inside budget |
| What does the year cost? | TCO base $131,800; high $139,000; adverse (caching broken) $136,800; model spend a tenth of total | 08 §2 | Inside budget |
| Can an injected attachment move data? | Trifecta: external leg cut (no send tool; adjuster sends); reader emits schema only; red-team 400 cases, ASR 1.2%, critical class 0 | 09 §2, §3, §5 | Critical class clean |
| Does the agent run with more rights than the adjuster? | Every tool call on the adjuster's token, scoped to the claim reference | 09 C-03, 06-integration-spec | Met |

## Conditions

1. Overall pass rate lower bound ≥ 0.80 on the full set. Owner: [claims quality lead]. Due: [date, before first customer-facing letter]. Route: fix the two lowest criteria or grow the set to 450 items.
2. Cost gate live in CI (fail any change raising cost per task by more than a tenth against 08 §1). Owner: [platform lead]. Due: [date, before shadow mode].
3. Approval override rate on bookings reported weekly; if it exceeds [rate] for two weeks, the auto-confirm rule is reviewed. Owner: [claims director].

## Accepted risks

| Risk | Control | Signed |
|---|---|---|
| An injected document biases a draft the adjuster does not catch | Output classifier (C-08); adjuster review; online judge on the attachment stratum at 100% | [legal director] |
| Provider capacity errors raise retries above the model's 4% | Gateway retry budget; fallback chain to be defined in 11-runbook.md | [platform lead] |

## What reopens this decision
A critical-class failure in any release; the adverse scenario's cost realised for two months; a new input source or tool (threat-model review trigger); the quarterly judge re-validation.
